Medical disclaimer

The Lantus dosing calculator on this site is an educational reference. It implements weight-based starting dose formulas and treat-to-target titration steps published in the ADA Standards of Medical Care in Diabetes and the Endocrine Society Clinical Practice Guideline. It does not replace the clinical judgment of a licensed prescriber.

No dose change should be made based on the calculator alone. Insulin requirements vary with kidney function, liver function, corticosteroid use, recent illness, meal patterns, pregnancy, and many other factors the calculator cannot evaluate. Always confirm dose adjustments with your physician, endocrinologist, pharmacist, or certified diabetes care and education specialist before injecting.

Not a substitute for emergency care

If you are experiencing hypoglycemia symptoms (confusion, sweating, shakiness, blurred vision), a fasting glucose below 70 mg/dL, or symptoms of severe hyperglycemia (fruity breath, vomiting, altered mental status), do not use this calculator. Contact your provider, a nurse line, or local emergency services.

Accuracy of calculations

Dosing algorithms are derived directly from published clinical guidelines. We cite the original sources on the calculator page and the About page so you can verify the logic. We review and update the formulas when the ADA or Endocrine Society publishes a guideline revision, but we cannot guarantee the output is appropriate for every clinical scenario.

Acceptable use

You may use the calculator and articles on this site for personal reference, professional practice, patient education, and academic teaching. You may share links to any page. You may not scrape or redistribute the content in bulk, re-host the calculator on another domain, or use automated clients to generate artificial traffic.

Limitation of liability
